Funcionalidad de la columna en pacientes tratados mediante artrodesis posterior con un sistema transpedicular en inestabilidad lumbosacra en el Hospital Miguel Hidalgo

Abstract

RESUMEN FUNCIONALIDAD DE LA COLUMNA EN PACIENTES TRATADOS MEDIANTE ARTRODESIS POSTERIOR CON UN SISTEMA TRANSPEDICULAR EN INESTABILIDAD LUMBOSACRA EN EL HOSPITAL MIGUEL HIDALGO. ANTECEDENTES: la espondilolistesis es una causa frecuente de dolor lumbar e incapacidad laboral así como del consumo crónico de analgésicos. JUSTIFICACION: existen pocos estudios realizados sobre la calidad de vida y funcionalidad de la columna lumbosacra a mediano y largo plazo, así como múltiples tratamientos quirúrgicos, con una alta prevalencia en la consulta de este hospital. OBJETIVO. Conocer el estado funcional de los pacientes con inestabilidad lumbosacra tratados por medio de artrodesis posterior con sistema transpedicular en el centenario hospital Miguel Hidalgo. MATERIAL Y MÉTODOS: Se estudiaron pacientes captados en la consulta externa con dicha patología y se realizaron cirugía en este hospital. RESULTADOS: Estudio transversal (crossover), en el Centenario Hospital Miguel Hidalgo entre los meses del mes de Julio del 2012 a Noviembre del 2013, con un total de 16 pacientes, de los cuales el 75% correspondió al sexo femenino y solo 4 hombres (25%), con un rango de edad entre 23 y 67 años con una media de 50.31 años. Se realizó seguimiento de los pacientes a las 2 semanas, 12 semanas, 6 meses y al año de seguimiento, fueron valorados por medio de la escala de Oswestry en la consulta pre quirúrgica y en las consultas subsecuentes, se realizó un análisis estadístico con la versión 15.0 Se analizó con la prueba de Friedman encontrando diferencia estadística entre las diferentes valoraciones (P 0.029), y con Wilcoxon a las dos semanas (p=0.7), a las 6 semanas (p=0.05), doce semanas (p=0.0.01), seis meses (P= < 0.001), y al año (p= < 0.001). CONCLUSIONES: Se encontró que los pacientes con una caja intersomatica mas artrodesis posterolateral con sistema transpedicular e injerto el índice de discapacidad se recuperó antes pero al final del estudio el índice de discapacidad fue similar. Se encuentra que la artrodesis posterolateral con sistema transpedicular es una buena opción en la inestabilidad degenerativa lumbosacra para mejorar el índice de discapacidad.

ABSTRACT COLUMN FUNCTION IN PATIENTS TREATED BY A SYSTEM TRANSPEDICULAR WITH ARTHRODESIS LUMBOSACRAL INSTABILITY IN THE HOSPITAL MIGUEL HIDALGO. BACKGROUND: spondylolisthesis is a common cause of low back pain and disability as well as the chronic use of analgesics. JUSTIFICATION: There are few studies on quality of life and functionality of the lumbosacral spine medium and long term as well as multiple surgical treatments, with a high prevalence in this hospital consultation. OBJECTIVE. Knowing the functional status of patients with lumbosacral instability treated by posterior fusion with transpedicular the centenary Miguel Hidalgo hospital system. MATERIAL AND METHODS: Patients recruited from outpatient surgery with this disease and were performed in this hospital were studied. RESULTS: Cross-sectional study (crossover) at Centennial Hospital Miguel Hidalgo between the months of July 2012 to November 2013, with a total of 16 patients, of whom 75 % were female and only 4 men ( 25 % ), with an age range between 23 and 67 years with a mean of 50.31 years. CONCLUSIONS: We found that patients with a more posterolateral interbody cage fusion with transpedicular grafting system and the disability index but recovered before the end of the study the rate of disability was similar. It is found that the posterolateral fusion with transpedicular system is a good choice in degenerative lumbosacral instability to improve disability index.
