RESUMEN
INTRODUCCIÓN: El estado de choque es el comportamiento clínico de la incapacidad circulatoria que resulta en uso inadecuado del oxígeno de las células, siendo de las primeras condiciones que aquejan a los pacientes de la Terapia Intensiva. Los predictores de respuesta hídrica, son considerados como piedra angular del diagnóstico y tratamiento dirigido.
OBJETIVO: Observar la respuesta a la fluidoterapia mediante la comparativa entre la Variabilidad de Presión de Pulso (VPP), Delta Down (ΔD) y Tiempo de Flujo aórtico corregido (TFc), en la Terapia Intensiva del Centenario Hospital Miguel Hidalgo.
METODOLOGÍA: Implicó la revisión de expedientes de pacientes con diagnóstico de estado de choque entre el periodo del 01 de febrero 2022 al 28 de febrero 2023; obteniendo los datos de género, edad, tipo de choque y la medición de los predictores de respuesta a volumen al tiempo de establecerse estado de choque y a las 48 horas de evolución del cuadro.
RESULTADOS: De 177 expedientes: 152 expedientes correspondieron al género masculino (85.9%), 25 pacientes al género femenino (14.1%). El tipo de choque diagnosticado fue: choque Séptico con 125 pacientes (70.6%); choque Hipovolémico con 42 pacientes (24.3%); choque Cardiogénico con 7 pacientes (4.0%); choque Neurogénico con 2 pacientes (1.1%). Se obtuvo la media de edad de 52.3 años. Dentro de las mediciones de los predictores de respuesta a volumen, se obtuvo que los pacientes fueron respondedores al momento del diagnóstico. Se observó que VPP no tuvo correlación con los pacientes respondedores a volumen; el predictor de volumen con mayor correlación fue TFc. De 118 pacientes totales positivos para respondedores con ΔD presentaron mejor evolución clínica.
CONCLUSIÓN: La variable de respuesta a volumen con un resultado significativo fue ΔD en comparación con otras variables. La VPP resultó menos útil en este estudio. Los pacientes reanimados de la Terapia Intensiva del Centenario Hospital Miguel Hidalgo no se beneficiaron de terapia hídrica a las 48 horas.

ABSTRACT
INTRODUCTION: The state of shock is the clinical behavior of circulatory incapacity that results in inadequate use of oxygen in the cells, being one of the first conditions that afflicts Intensive Care patients. Predictors of fluid response are considered the cornerstone of diagnosis and targeted treatment.
OBJECTIVE: To observe the response to fluid therapy by comparing Pulse Pressure Variability (PPV), Delta Down (ΔD) and Corrected Aortic Flow Time (TFc), in the Intensive Care Unit of the Centenario Hospital Miguel Hidalgo.
METHODOLOGY: It involved the review of records of patients with a diagnosis of shock between the period from February 1, 2022 to February 28, 2023; obtaining data on gender, age, type of shock and measuring the predictors of response to volume at the time of establishing shock and 48 hours after the onset of the condition.
RESULTS: Of 177 files: 152 files corresponded to the male gender (85.9%), 25 patients to the female gender (14.1%). The type of shock diagnosed was: Septic shock with 125 patients (70.6%); Hypovolemic shock with 42 patients (24.3%); Cardiogenic shock with 7 patients (4.0%); Neurogenic shock with 2 patients (1.1%). The average age was 52.3 years. Among the measurements of the predictors of response to volume, it was obtained that the patients were responders at the time of diagnosis. It was observed that PPV had no correlation with volume responsive patients; The volume predictor with the highest correlation was TFc. Of 118 total patients positive for responders with ΔD, they presented better clinical evolution.
CONCLUSION: The volume response variable with a significant result was ΔD compared to other variables. PPV was less useful in this study. The resuscitated patients from the Intensive Care Unit of the Centenario Hospital Miguel Hidalgo did not benefit from water therapy after 48 hours.
