Caracterización clínica en pacientes neonatos con COVID-19 en una unidad de segundo nivel

Abstract

RESUMEN. TÍTULO: “CARACTERIZACIÓN CLÍNICA EN PACIENTES NEONATOS CON COVID-19 EN UNA UNIDAD DE SEGUNDO NIVEL”. ANTECEDENTES: La salud pública mundial, internacional, de regiones y países consideran las dos dimensiones fundamentales: las condiciones de salud de las poblaciones y la respuesta social a través de los sistemas de salud. Los investigadores han descrito informes de caso sobre pacientes pediátricos como neonatos encontrando hallazgos clínicos, resultados de laboratorio y radiológicos para caracterizar la enfermedad de COVID-19. OBJETIVO: Identificar la distribución y frecuencia de las características sociodemográficas, clínicas, de laboratorio e imagenología, así como la clasificación de la enfermedad de pacientes neonatos con COVID-19 en una unidad de segundo nivel delegación Aguascalientes durante la pandemia de COVID-19. MATERIAL Y MÉTODOS: Estudio transversal simple, con un muestreo por conveniencia, en donde se contemplarán a la totalidad de los ingresos de pacientes neonatos con diagnóstico de COVID-19 de marzo 2020 a marzo 2022. Para el análisis estadístico se utilizará estadística descriptiva, en las variables cualitativas frecuencias y porcentajes, para variables continuas se utilizaron medidas de tendencia central y dispersión como la mediana y los rangos intercuartílicos. RESULTADOS: La muestra total de neonatos en el periodo de estudio fue de 16. Las madres de los neonatos con COVID-19 ya tenían la infección antes del nacimiento en un 25%. Los neonatos tuvieron una edad gestacional de 33.1 semanas por Capurro, con un promedio de peso de 1.639 kg y un APGAR de 6-7 en el 60%, los síntomas más frecuentes fueron irritabilidad, disnea y fiebre en más del 50%. CONCLUSIONES: A pesar de tener un número pequeño de muestra, se obtuvieron datos clínicos importantes en esta población de estudio, que definitivamente tiene un comportamiento muy distinto a la población adulta, siendo los hallazgos más importantes el nacimiento pretérmino, peso bajo y APGAR menor al normal. En cuanto a las características clínicas, al igual que otros autores, la fiebre y la disnea fueron los más frecuentes. RECURSOS E INFRAESTRUCTURA: Los recursos materiales y financieros necesarios para la realización de esta investigación serán proporcionados por los investigadores participantes. EXPERIENCIA DE LOS INVESTIGADORES: la investigadora responsable tiene experiencia en dirección de protocolos de investigación, la co-investigadora tiene experiencia en atención a pacientes neonatos y el investigador asociado terminó la residencia de Medicina Familiar. ABSTRACT TITLE: Clinical characterization in neonate patients with COVID-19 in a second level unit. BACKGROUND: Global, international, regional and country public health considers two fundamental dimensions: the health conditions of populations and the social response through health systems. Investigators have described case reports on pediatric patients such as neonates finding clinical, laboratory, and radiological findings to characterize COVID-19 disease. OBJECTIVE: To identify the distribution and frequency of sociodemographic, clinical, laboratory and imaging characteristics, as well as the classification of the disease of neonatal patients with COVID-19 in a second-level Aguascalientes delegation unit during the COVID-19 pandemic. MATERIAL AND METHODS: simple cross-sectional study, with a convenience sample, where all admissions of neonatal patients diagnosed with COVID-19 from January 2020 to December 2021 will be considered. For the statistical analysis, descriptive statistics will be used, in qualitative variables frequencies and percentages, for continuous variables measures of central tendency and dispersion will be used, depending on the distribution of the data, if the mean and standard deviation are normal, otherwise the median and interquartile ranges. RESULTS: The total sample of newborns in the study period was 16. The mothers of the newborns with COVID-19 had the infection before birth in 25%. The neonates had a gestational age of 33.1 weeks per capurro, with an average weight of 1.639 kg and an APGAR of 6-7 in 60%, the most frequent symptoms were irritability, dyspnea and fever in more than 50%. CONCLUSIONS: despite having a small sample number, important clinical data were obtained in this study population, which definitely behaves very differently form the adult population, the most important findings being preterm birth, low weight and APGAR less than normal. Regarding the clinical characteristics like other authors, fever and dyspnea were the most frequent in the new born. RESEARCHERS EXPERIENCE: the lead researcher has experience directing research protocols, the co-investigator has experience caring for neonatal patients, and the associate researcher completed the Family Medicine residency.
