Tasa de éxito clínico y radiográfico tras artrodesis de tobillo por artropatía de Charcot

Abstract

INTRODUCCIÓN: La Artropatía de Charcot (AC) es una de las complicaciones neurovasculares más infradiagnosticadas en el paciente diabético mal controlado. Su tratamiento representa un desafío para el cirujano ortopedista y sus complicaciones se enlistan en la literatura como de las más difíciles de tratar. OBJETIVO: Determinar la Tasa de éxito clínico y radiográfico tras artrodesis de tobillo por artropatía de Charcot operados en el módulo de pie y tobillo del servicio de Traumatología y Ortopedia. METODOLOGÍA: Estudio retrospectivo, comparativo y analítico en pacientes atendidos por el servicio de Traumatología y Ortopedia en el Centenario Hospital Miguel Hidalgo con diagnóstico de AC tratados quirúrgicamente con artrodesis con clavo centromedular, placa de artrodesis, fijador externo o grapas. RESULTADOS: Se analizaron un total de 21 pies con una media de edad al momento del tratamiento de 50.7 años. 11 de ellas son mujeres y 10 hombres. De los pacientes incluidos, 9 pies fueron de lado derecho y 12 pies izquierdos. De acuerdo al tipo de artrodesis según la anatomía se encontraron 6 tipos siendo la más frecuente la calcáneo-tibio-astragalina en 10 pies seguida en frecuencia de la tibioastragalina (23.8%), la triple artrodesis (14.3%), y en un paciente respectivamente la metatarsofalángica, cuneoescafoidea y astrágalocuneana con un 4.8% cada una. De acuerdo a los implantes utilizados en el estudio, por orden de frecuencia se analizaron 8 pies tratados con enclavado centromedular con clavo especial para artrodesis, 11 pies tratados con tornillos canulados, 1 paciente artrodesado con grapas y otro más artrodesado con placa LCP autobloqueante. De las complicaciones, 11 pacientes no presentaron ninguna complicación y reportaron una evolución natural del postquirurgico. Sin embargo, de los otros 10 pacientes, 4 presentaron fuga del clavo centromedular de artrodesis de los cuales en todos los casos se retiró el clavo centromedular, en 2 de ellos se colocó un fijador externo hasta alcanzar la consolidación y en los otros 2 se manejó una pseudoartrosis funcional; 3 presentaron infección del sitio quirúrgico. Actualmente la mayoría se encuentran apoyando con alguna ortesis. CONCLUSIONES: La artrodesis en la AC es reproducible en nuestro hospital y con resultados clínicos satisfactorios para los pacientes, por lo que la elección del implante según el área a artrodesar en el pie es la decisión más importante a tomar para ofrecerle un mejor pronóstico al paciente.

INTRODUCTION: Charcot Arthropathy (CA) is one of the most underdiagnosed neurovascular complications in poorly controlled diabetic patients. Its treatment represents a challenge for the orthopedic surgeon and its complications are listed in the literature as one of the most difficult to treat. OBJECTIVE: To determine the clinical and radiographic success rate after ankle arthrodesis due to Charcot arthropathy operated on in the foot and ankle module of the Traumatology and Orthopedics service. METHODOLOGY: Retrospective, comparative and analytical study in patients attended by the Traumatology and Orthopedics service at the Centenario Hospital Miguel Hidalgo with a diagnosis of CA treated surgically with arthrodesis with a centromedullary nail, arthrodesis plate, external fixator or staples. RESULTS: A total of 21 feet were analyzed with a mean age at the time of treatment of 50.7 years. 11 of them are women and 10 men. Of the included patients, 9 feet were on the right side and 12 feet on the left. According to the type of arthrodesis according to the anatomy, 6 types were found, the most frequent being the calcaneal-tibio-talar in 10 feet followed in frequency by the tibiotalar (23.8%), the triple arthrodesis (14.3%), and in one patient respectively. metatarsophalangeal, cuneoscaphoid, and talo-cuneal with 4.8% each. According to the implants used in the study, in order of frequency, 8 feet treated with centromedullary nailing with a special nail for arthrodesis, 11 feet treated with cannulated screws, 1 patient arthrodesized with staples and another arthroderated with a self-locking LCP plate were analyzed in order of frequency. Of the complications, 11 patients did not present any complications and reported a natural post-surgical evolution. However, of the other 10 patients, 4 presented leakage of the centromedullary arthrodesis nail of which in all cases the centromedullary nail was removed, in 2 of them an external fixator was placed until consolidation was achieved and in the other 2 it was managed a functional nonunion: 3 presented infection of the surgical site. Currently most are supporting with an orthosis. CONCLUSIONS: Arthrodesis in CA is reproducible in our hospital and with satisfactory clinical results for patients, so the choice of the implant according to the area to be arthrodes in the foot is the most important decision to take to offer a better prognosis to the patient.
