Manejo de sonda endopleural blanda en neumotórax y derrame pleural contra sonda endopleural rígida

Abstract

Introducción: Clásicamente se utilizan sondas rígidas para el manejo de neumotórax y derrames pleurales, la mayoría de estos pacientes tienen un manejo intrahospitalario solo justificado por el sello de agua y la necesidad de mayor cantidad de analgésicos por el tubo rígido endopleural. Objetivo: Determinar que el uso de sondas pleurales blandas en pacientes con neumotórax y derrame pleural disminuye el dolor y los días de estancia intrahospitalaria en el Centenario Hospital Miguel Hidalgo. Métodos: Se revisaron los expedientes clínicos del Hospital Centenarios Hospital Miguel Hidalgo identificando los pacientes sometidos a colocación de sonda endopleural blanda y rígida en el periodo 2018 a 2021. Se incluyeron a todos los pacientes mayores de 18 años manejados para trauma abierto y cerrado de tórax leve, cirugía torácica, derrame pleural maligno y de otras índoles. Resultados: Se incluyeron un total de 85 pacientes, 55 (65%) pacientes fueron mujeres y 30 (35%) hombres, %). La mediana de edad para Blake fue de 48 años (rango 10-73) y para sonda Argyl es 34 años (rango 18-80). Los días de estancia intrahospitalaria para los sondas Blake fue menor a la sonda Argyl (p 0.024, IC 95%), los días de uso de sonda Blake fue mayor que la sonda Argyl (p 0.007, IC 95%), la necesidad de analgésicos para sonda Blake fue menor que la sonda Argyl (p 0.005, IC 95%) Conclusiones: Se identificó que el uso de sonda Blake tiene una menor necesidad de días de estancia intrahospitalaria así como de analgésicos Palabras claves: Derrame pleural, neumotórax, hemotórax, sonda endopleural

Introduction: Rigid probes are traditionally used for the management of pneumothorax and pleural effusions; most of these patients have in-hospital management only justified by the water seal and the need for a greater amount of analgesics due to the rigid endopleural tube. Objective: To determine that the use of soft pleural tubes in patients with pneumothorax and pleural effusion reduces pain and length of hospital stay at the Centenario Hospital Miguel Hidalgo. Methods: The clinical records of the Hospital Centenarios Hospital Miguel Hidalgo were reviewed, identifying the patients who underwent placement of a soft and rigid endopleural tube in the period 2018 to 2021. All patients over 18 years of age managed for open and closed mild chest trauma were included, thoracic surgery, malignant pleural effusion and other types. Results: A total of 85 patients were included, 55 (65%) patients were women and 30 (35%) men (%). The median age for Blake was 48 years (range 10-73) and for Argyl catheter it was 34 years (range 18-80). The days of hospital stay for the Blake catheters were less than the Argyl catheter (p 0.024, 95% CI), the days of use of the Blake catheter was greater than the Argyl catheter (p 0.007, 95% CI), the need for analgesics for Blake catheter was lower than Argyl catheter (p 0.005, 95% CI) Conclusions: It was identified that the use of the Blake catheter has a lower need for hospital stay days as well as analgesics Key words: pleural effusion, pneumothorax, hemopneumothorax, endopleural tube.
