Efectividad en el manejo inicial de los pacientes quemados atendidos en el Centenario Hospital Miguel Hidalgo durante el periodo del 2014-2019

Abstract

RESUMEN ANTECEDENTES: las quemaduras, son lesiones causadas por diversos agentes físicos que generan lesiones que van desde eritema hasta la destrucción total de estructuras dérmicas y debajo de la dermis. La mejoría en la mortalidad se debe a progresos en la compresión de la fisiopatología de la lesión térmica, soporte nutricional, escisión e injertos de manera temprana, restricción en el uso de antibiótico profiláctico, etc. Sin embargo a pesar de conocer dichas medidas el pronóstico del paciente gran quemado continua siendo sombrío, esto se debe a la condición clínica del paciente per se. Sin embargo, si aunado a esto, no aplicamos estas estrategias, ya conocidas, el pronóstico del paciente se verá seriamente afectado. Una de las guías a nuestro alcance es la Guía de Práctica Clínica Mexicana del Paciente ”Gran quemado”, la cual otorga 29 recomendaciones para estandarizar y optimizar el manejo en estos pacientes. OBJETIVO GENERAL: determinar la efectividad del manejo inicial en los pacientes quemados atendidos en el Centenario Hospital Miguel Hidalgo durante el periodo del 2014 – 2019. MATERIAL Y MÉTODOS: se realizó un estudio observacional, retrospectivo, transversal y descriptivo en los pacientes con quemaduras atendidos en el Centenario Hospital Miguel Hidalgo en el periodo de 2014-2019. Se analizaron los casos y se consideró un manejo efectivo si se cumplió con el 80% de las recomendaciones establecidas en la Guía de Práctica clínica Mexicana del paciente “Gran Quemado”. RESULTADOS: se realizó el análisis de la aplicación de los criterios de la Guía de Práctica Clínica del paciente “Gran Quemado” en los 65 pacientes quemados que se manejaron en el periodo antes mencionado, encontrando que las recomendaciones establecidas en la Guía se cumplen apenas en el 75%. CONCLUSIONES: con base en los resultados adoptamos la hipótesis nula que establece que el manejo inicial de los pacientes en el Centenario Hospital Miguel Hidalgo no es efectivo.
