Introducción. El desarrollo de la falla renal aguda en los pacientes neonatales ha sido un reto continuo desde el surgimiento de las terapias intensivas neonatales. Esto ha orillado a crear estrategias para la detección oportuna y técnicas específicas para el manejo de la misma. El desarrollo de estrategias para identificar factores de riesgo puntuales ha sido un hito en la neonatología, el presente estudio intenta definir como factor puntual el manejo de líquidos y balance hídrico como determinante del desarrollo de falla renal aguda en nuestra terapia intensiva neonatal.
Material y métodos. Se captaron los pacientes internados en la terapia intensiva neonatal en el periodo comprendido entre 1 de enero del 2010 al 30 de diciembre del 2014. Se incluyeron pacientes ingresados en la UCIN durante dicho periodo con estancia mayor a 72 horas, excluyendo pacientes sin hoja de balance hídrico, con daño renal previamente establecido, pacientes con terapia sustitutiva previa, con diagnóstico de gastrosquisis. Utilizando para el estudio programas básicos: Descriptivos y frecuencias (medidas de tendencia central y porcentaje) Análisis univariado : Prueba exacta de Fisher, Kaplan Meier ( Chi cuadrada - variable tiempo Log Rank ) , t de Student. Modelo Lineal Univariado (ANOVA DE DOS FACTORES) Análisis multivariado: Regresión logística binaria con nivel de significancia <0.05.

Resultados. Fueron incluidos en el estudio un total de 130 pacientes (n), quedando excluidos 6 por no cumplir con criterios, del total 48 (38.7%) fueron del sexo femenino y 78 (61.3%) del sexo masculino. El promedio de la edad gestacional fue 35.3 ± 3,2. El promedio de estancia hospitalaria fue de 15 días ±9.7, en relación al balance hídrico global resultó de -1.38 ml ± 50 ml ( Rango -184 ml a 127 ml) corroborado en relación a hoja de balance hídrico ofrecido por la UCIN. La presencia de daño renal agudo se presentó en el 18.5% de los pacientes.
Conclusión. El balance hídrico negativo en los pacientes recién nacidos, es un factor de riesgo independiente para desarrollar insuficiencia renal aguda en la etapa neonatal. El impacto no se visualiza porque la gran mayoría de los pacientes en este periodo de vida cuando se presenta daño renal agudo, son pacientes no oligúricos.
Introduction. The development of acute renal failure in neonatal patients has been a continuous challenge since the emergence of neonatal intensive therapy. This has led to the creation of strategies for timely detection and specific techniques for managing it. The development of strategies to identify specific risk factors has been a milestone in neonatology, the present study attempts to define as a point factor the management of fluids and water balance as a determinant of the development of acute renal failure in our neonatal intensive care. Univariate Linear Model (TWO-FACTOR ANOVA) Multivariate analysis: Binary logistic regression with significance level <0.05.

Results. A total of 130 patients (n) were included in the study, 6 were excluded because they did not meet criteria, 48 (38.7%) were female and 78 (61.3%) were male. The mean gestational age was 35.3 ± 3.2. The mean hospital stay was 15 days ± 9.7, in relation to the global water balance, it was -1.38 ml ± 50 ml (Range -184 ml to 127 ml) corroborated in relation to the water balance sheet offered by the NICU. The presence of acute renal damage was present in 18.5% of the patients. Conclusion. Negative fluid balance in newborn patients is an independent risk factor for developing acute renal failure in the neonatal stage. The impact is not visualized because the great majority of patients in this period of life when acute renal damage is present, are non-oliguric patients.
