Estrategia restrictiva de transfusión sanguínea en el Hospital de la Mujer Aguascalientes

Abstract

RESUMEN: Apegarse a lineamientos restrictivos de trasfusión sanguínea, fundamentados en evidencia clínica, optimiza el uso de hemoderivados en general, disminuyendo su empleo innecesario y los riesgos propios a su uso, indicándose solo en casos donde el beneficio sobrepase los riesgos que conlleva su utilización. Objetivo general: Describir el apego a una estrategia restrictiva de transfusión sanguínea en el Hospital de la Mujer Aguascalientes. Diseño y métodos: Se realizó un estudio observacional, descriptivo, retrospectivo en el enero a diciembre del 2018, en pacientes obstétricas que hubieran recibido trasfusiones sanguíneas en el Hospital de la Mujer. Resultados: Se indicó la terapia transfusional a un total de 471 pacientes, la edad promedio fue 24.7 años (DE ± 6.8), y una media de hemoglobina al ingreso, de 10.2 g/dL (IC 95%, 9.4-10.9 g/dl, DE ± 2.3). El volumen sanguíneo perdido medio fue 818ml (p<0.001), el nivel medio de Hb al egreso fue 9.7 g/dL, (IC 95%, 9.3-10.1 g/dL, DE ± 1.18), el aumento en los niveles de Hb fue estadísticamente significativo posterior a la transfusión (p<0.001). El nivel hemoglobina al egreso se mantiene arriba de 9 g/dL, tienen mayores niveles de hemoglobina los pacientes con hemoglobina al ingreso <7 g/dL (Hb 9.99, IC 95% 9.13-10.85). Conclusiones: La política de transfusión sanguínea en el Hospital de la Mujer Aguascalientes no sigue lineamientos restrictivos. Solo el 4% de pacientes en quien se indican transfusiones múltiples, reciben una evaluación integral del estado clínico previo a la terapia transfusional. ABSTRACT: The use of restrictive blood transfusion guidelines, based on clinical evidence, optimizes the use of blood products in general, reducing the unnecessary use and the risks associated with its administration, transfusing only in cases where the benefit outweighs the risks involved in its use. Objective: Describe the attachment to a restrictive blood transfusion strategy at the Aguascalientes Women's Hospital. Design and methods: An observational, descriptive, retrospective study was conducted from January to December 2018, in obstetric patients who have received blood transfusions at the Women's Hospital. Results: Transfusion therapy is indicated for a total of 471 patients, the mean age was 24.7 years (SD ± 6.8), with an average hemoglobin level at admission of 10.2 g/dl (95% CI, 9.4-10.9 g/dl, DE ± 2.3). The mean blood lost volume was 818ml (p <0.001), the average Hb level at discharge was 9.7 g/dl, (95% CI, 9.3-10.1 g/dl, SD ± 1.18), the increase in Hb levels It has significance (p <0.001). The hemoglobin level at discharge is maintained above 9 g/dl, patients with hemoglobin have higher hemoglobin levels at admission <7 g/dl (Hb 9.99, 95% CI 9.13-10.85). Conclusions: The blood transfusion policy at the Aguascalientes Women's Hospital does not follow restrictive guidelines. Just 4% of patient who multiple transfusions were indicated had a comprehensive evaluation of the clinical status prior to transfusion therapy.
