Uso de parches de nicotina para la prevención de cefalea post punción no intencionada de duramadre en el Centenario Hospital Miguel Hidalgo

Abstract

INTRODUCCIÓN. La cefalea post punción de duramadre(CPPD), es una complicación frecuente de la anestesia neuroaxial que, aunque no grave, puede ser incapacitante para el paciente. El tratamiento conservador de la CPPD tiene un porcentaje de éxito bajo. Existe evidencia de que el hábito tabáquico protege al paciente para desarrollar CPPD, pero no se ha comprobado si este efecto se debe a la nicotina y no hay investigaciones previas que aborden este tema. OBJETIVO. Comparar y evaluar en dos grupos de estudio, uno usando parches de nicotina transdérmico y el otro control, el desarrollo de CPPD posterior a punción no intencional de duramadre. MATERIALES Y MÉTODOS. Ensayo clínico comparativo no aleatorizado de casos y controles de pacientes sometidos a anestesia regional que sufrieron punción no intencional de duramadre en el Hospital Hidalgo. El grupo de estudio recibe un parche de nicotina transdérmico posterior a la punción y el grupo control no. A las 24 horas de la punción se valoró la presencia de CPPD, si no se presentó, el paciente egresó a su domicilio con un teléfono de reporte para notificar si desarrolló CPPD dentro de los siguientes 7 días. RESULTADOS. Se incluyeron 11 pacientes, 5 controles y 6 del grupo de nicotina; 45.45% de ellos desarrollaron cefalea post punción no intencionada de duramadre. Encontramos que no existe relación entre la edad y el sexo del paciente y el desarrollo de cefalea post punción de duramadre. El tratamiento con nicotina no parece impactar en la aparición de los síntomas de cefalea post punción no intencionada de duramadre en este grupo de pacientes.

INTRODUCTION: The Post Puncture Headache of Dura Mater is a complication very frequent in the anesthesia neuroaxial that is not serious, but it can be a reason of incapacity for ta patient. The regular treatment for this problem it’s conservative and with a very low percentage in the results. There is some evidence that the smoke habit protects the patient against the TPPHDM. But there are not previous investigations about the nicotine and the TPPHDM. OBJECTIVE: To compare two groups. One, using patches of transdermal nicotine, and other one as a control of the TPPHDM. MATERIALS AND METHODS: Clinic comparative essay, not random, of patients submitted to regional anesthesia that suffer TPPHDM in Hidalgo Hospital The group of the study receive a patch and the control group not. 24 hours we check that the group with the patches is ok. If TPPHDM do not appear, we wait for notices after one week. The patients have our number for calling us. RESULTS: The sample were of eleven patients, 6 in the nicotine patches study group and 5 in the control group. In the study group 45.45% developed TPPHDM. We discover that there are not links between the ages and sex of the patients for the TPPHDM. The nicotine is not really an element that impact in the TPPHDM in this group of patients.
