Abstract

1. RESÚMEN TOMA DE DECISIONES MÉDICAS PARA INGRESO EN LA UNIDAD DE CUIDADOS INTENSIVOS E IMPACTO ECONOMICO EN EL CENTENARIO HOSPITAL MIGUEL HIDALGO García Sandoval Alejandra Monserrat, Martínez Cano José Salvador. Castillo González Roberto Alejandro. Gutierrez Martínez Silvia Patricia INTRODUCCIÓN. La responsabilidad que supone desempeñar el papel de elector de uno entre varios pacientes “elegibles” para ser admitidos en las Unidades de Cuidados Intensivos es enorme, por cuanto equivale a establecer cuál de esas personas debe recibir mejor oportunidad de continuar viviendo, ocasionalmente en la práctica no existen posibilidades de ofrecerles la asistencia requerida por carecer de los recursos indispensables, tanto en recursos materiales como económicos. Así, uno de los dilemas más trascendentales que enfrenta el médico intensivista contemporáneo es cuándo decidir el ingreso de un paciente en la UCI, y una vez ingresado y tratado con una serie de técnicas de soporte vital, si la evolución no resulta satisfactoria, por cuánto tiempo mantenerlas. Tales decisiones poseen importantes implicaciones éticas y económicas ya que según la evolución al paciente se le puede llegar a someter a un largo y costoso proceso ya sea para su egreso de la unidad o antes de morir. OBJETIVO. Evaluar el impacto que tiene una adecuada selección de pacientes candidatos a ingresar a la unidad de cuidados intensivos, tanto en lo económico como días de estancia. METODOLOGÍA. El presente estudio es observacional, prospectivo, descriptivo, y longitudinal. Incluyendose todos los pacientes que se ingresan a la unidad de cuidados intensivos (unidad de cuidados intensivos intermedios asi como unidad de cuidados intensivos generales) evaluando el motivo del ingreso del paciente a la unidad, tipo de prioridad, patología de ingreso y los días de estancia así como el motivo del egreso. RESULTADOS. Se observó que aquellos pacientes que no tuvieron una valoración previa a la unidad de cuidados intensivos, y que su admisión fue de tipo administrativo, se prolongaron los días de estancia y por lo tanto el incremento de costos de estadía. CONCLUSIONES. Es importante evaluar de manera objetiva a cada uno de los pacientes que se ingresan a la Unidad de cuidados Intensivos, ya que de esto depende la estancia en la unidad, y por lo tanto los costos que se generan para la institución. The responsibility that comes with playing the role of one voter between patients "eligible" for admission to the intensive care units is huge, because it is equivalent to establish which of these people should get better chance to continue living, occasionally in practice there is no possibility to offer the assistance required by lack of essential resources, both material and financial resources. So, one of the most significant dilemmas facing contemporary intensive care physician is deciding when a patient was admitted to the ICU, and once admitted and treated with a series of life support, if evolution is not satisfactory, how keep time. Such decisions have important ethical implications and economic developments since according to patient can get to undergo a long and costly process either for or discharge from the unit before dying. OBJECTIVE. Evaluate the impact of appropriate selection of patients eligible to enter the intensive care unit, both economically and days of stay. METHODOLOGY. The present study is an observational, prospective, descriptive, and longitudinal. Including all patients admitted to the ICU (intensive care unit as well as intermedium and general intensive care unit) evaluating the patient's reason for admission to the unit, priority type, income pathology and days of stay and the reason for the discharge. RESULTS. It was observed that patients who did not have an evaluation prior to the intensive care unit, and that its admission was administrative, lasted the length of stay and therefore increased costs of Stay. CONCLUSIONS. It is important to objectively evaluate each of the patients admitted to the Intensive Care Unit, as this depends on the unit stay, and therefore the costs are generated for the institution.
