RESUMEN
En américa las enfermedades cardiovasculares son la principal causa de muerte. La enfermedad arterial coronaria se relaciona con factores de riesgo cardiovascular como hipertensión arterial, diabetes mellitus, dislipidemia, tabaquismo y obesidad. Desde hace años se conoce la relación que existe entre la enfermedad arterial coronaria con el signo de Frank, siendo este, un indicador temprano de enfermedad arterial coronaria (pacientes sin síntomas), pudiendo tener impacto su hallazgo en la aplicación de medidas de prevención para el síndrome coronario agudo. La importancia del SF o pliegue en lóbulo de la oreja (PLO) radica en que la oreja y corazón comparten el mismo suministro sanguíneo a través de arterias terminales, pudiendo ser un reflejo a nivel cutáneo del grado de lesión de las arterias coronarias. Existen múltiples estudios de la asociación del SF con la EAC. Sin embargo, hay pocos o nulos estudios donde se evalúe el SF como un predictor de la carga ateroesclerótica. Se sabe que la diabetes mellitus, la dislipidemia y el tabaquismo se asocian con la enfermedad multivaso, pero, no hay un signo clínico que sea predictor de enfermedad coronaria multivaso. Nuestro estudio valora la relación del SF con la carga ateroesclerótica (número de vasos afectados) mediante angiografía coronaria en pacientes que ingresaron con diagnóstico de SICA al Centenario Hospital Miguel Hidalgo o lo presentaron durante su estancia, encontrando estrecha relación del SF con la EAC multivaso; teniendo un considerable valor predictivo negativo la ausencia del signo para la enfermedad trivascular.; ABSTRAC
In America, cardiovascular diseases are the number one cause of death. Coronary artery disease is related to diabetes mellitus, hypertension, obesity and dyslipidemia among others. The relationship between coronary artery disease and Frank's sign has been known for decades, this being an early indicator of coronary artery disease (patients without symptoms), and its finding may have an impact on the application of preventive measures for coronary syndrome acute. The importance of the SF or earlobe fold lies in the fact that the ear (PLO) and the heart share the same blood supply through terminal arteries, and may be a reflection at the cutaneous level of the degree of damage to the coronary arteries. There are multiple studies of the association of Frank's sign with coronary artery disease. However, there are few or no studies where SF is evaluated as a predictor of atherosclerotic burden. Diabetes mellitus, dyslipidemia, and smoking are known to be associated with multivessel disease, but there is no clinical sign that is a predictor of multivessel coronary disease. Our study assesses the relationship of SF with atherosclerotic burden (number of vessels affected) by coronary angiography in patients admitted with a diagnosis of acute coronary syndrome to the Centenario Hospital Miguel Hidalgo or presented during their stay, observing a significant association of Frank's sign with multivessel coronary disease; having a considerable negative predictive value the absence of the sign for trivascular disease.

RESUMEN
Antecedentes: La enfermedad por coronavirus 2019 ocasiono un desafío en la atención médica. Aproximadamente el 20% de los pacientes amerito ventilación mecánica. Es poco lo que se conoce de la supervivencia de estos pacientes.
Objetivo: Comparar la supervivencia de pacientes con COVID-19 Grave que no aceptaron ventilación mecánica invasiva con la supervivencia de pacientes con COVID-19 Grave que aceptaron ventilación mecánica invasiva en el servicio de Medicina Interna del CHMH de febrero del 2021 a febrero del 2022.
Metodología: Se obtuvo una base de datos de los pacientes hospitalizados en el Hospital Centenario Miguel Hidalgo con COVID 19 grave que no aceptaron ventilación mecánica invasiva, se obtuvo un total de 45 pacientes y 3 fueron excluidos, se realizó emparejamiento por edad y sexo 1 a 2 con el grupo que si acepto ventilación mecánica invasiva, con una muestra final de 126 pacientes, se realizó análisis estadístico con el programa STATA®, se calculó media, mediana y rango intercuartílico para análisis univariado, para la comparación de los grupos se realizó prueba t de student y prueba exacta de Fisher y U de Mann Withney, para el análisis de supervivencia se realizó Kaplan-Meier, así como análisis multivariado utilizando regresión de Cox y de Poisson.
Resultados: En ambos grupos predomino el sobrepeso y obesidad grado I, se documentó mayor tiempo de estancia hospitalaria en el grupo que acepto intubación (18 ± 18.2 vr 12 ± 10.8), el tiempo entre inicio de síntomas y desenlace fue diferente, el tiempo entre aceptar o rechazar intubación y desenlace fue mayor para el grupo que acepto intubación, el grupo que no acepto intubación tuvo más comorbilidades, el grupo que acepto intubación desarrollo más lesión renal aguda, requerimiento de hemodiálisis y uso de vasopresores. No hubo diferencias estadísticamente significativas para la supervivencia en ambos grupos, pero si se evidencio diferencia en el desarrollo de lesión renal aguda.
Conclusiones: No hubo diferencia en la supervivencia en cuanto aceptar o no intubación orotraqueal, pero desarrollar lesión renal aguda disminuyo el riesgo de supervivencia en ambos grupos.

Background: Coronavirus disease 2019 has created a challenge in healthcare. Approximately 20% of patients require mechanical ventilation. Little is known about the survival of these patients.
Objective: Compare the survival of patients with Severe COVID-19 who did not accept invasive mechanical ventilation with the survival of patients with Severe COVID-19 who accepted invasive mechanical ventilation in the Internal Medicine service of the CHMH from February 2021 to February 2022.
Methodology: A database of patients hospitalized at the Hospital Centenario Miguel Hidalgo with severe COVID 19 who did not accept invasive mechanical ventilation was obtained, a total of 45 patients were obtained and 3 were excluded, matching was performed by age and sex 1 to 2 with the group that accepts invasive mechanical ventilation, with a final sample of 126 patients, statistical analysis was carried out with the STATA® program, mean, median and interquartile range were calculated for univariate analysis, for the comparison of the groups a test was performed Student's t and Fisher's exact test and Mann Withney's U, Kaplan-Meier was performed for survival analysis, as well as multivariate analysis using Cox and Poisson regression.
Results: In both groups, overweight and obesity grade I predominated, a longer hospital stay was documented in the group that accepted intubation (18 ± 18.2 vr 12 ± 10.8), the time between onset of symptoms and outcome was different, the time between accepting or rejecting intubation and outcome was higher for the group that accepted intubation, the group that did not accept intubation had more comorbidities, the group that accepted intubation developed more acute kidney injury, required hemodialysis and use of vasopressors. There were no statistically significant differences for survival in both groups, but a difference was evident in the development of acute kidney injury.
Conclusions: There was no difference in survival in accepting or not accepting orotracheal intubation, but developing acute kidney injury decreased the risk of survival in both groups.

Epidemiología de las infecciones en pacientes pediátricos con cáncer y neutropenia febril atendidos en el Centenario Hospital Miguel Hidalgo

RESUMEN
INTRODUCCION. La Neutropenia Febril es la complicación más frecuente y con mayor impacto en la supervivencia global. El manejo debe individualizarse considerando los reportes epidemiológicos del hospital y la variabilidad del tipo de bacterias que se aíslan a través del tiempo y la aparición de bacterias multirresistentes. OBJETIVO. El objetivo de este estudio fue identificar microorganismos aislados, tipo de cultivo y asociación con el diagnóstico oncológico. METODOLOGÍA. Se incluyeron eventos de NF ingresados a Urgencias Pediátricas de 2016 a 2022. Se utilizó estadística descriptiva con el software SPSS v24 y análisis inferencial. RESULTADOS. Se registraron 580 eventos con 16 % de cultivos positivos, 44.1 % correspondieron al sexo masculino y 55.9 % femenino, de 1 a 17 años. Los diagnósticos con mayor porcentaje de cultivos reportados positivos fueron LLA 51 eventos (54.8 %) y LMA 20 eventos (21.5 %). Por año analizado hay predominio en los años 2018 y 2019 con 24.7 y 24.2 % respectivamente y el 2021 con 8.7% siendo el registro menor. El urocultivo fue reportado positivo en 40.9 %, hemocultivo periférico con 31.2 %, hemocultivo central 16.1 %, cultivo de secreciones 8.6 %, secreciones bronquiales 2.2 % y biopsia 1.1 %. Los microorganismos reportados mayormente fueron GN en un 86 % y 13 GP, en mayor porcentaje Pseudomonas aeruginosa en 22.6 %, Escherichia coli 24.7 % y Escherichia coli BLEE + con 17.2 %. CONCLUSIÓN. La población estudiada reflejó características acordes en cuanto a edad y diagnóstico con mismas tendencias epidemiológicas a lo descrito usualmente. El total de cultivos positivos se apega al 15 al 20 % reportado. Con mayor prevalencia de GN, Escherichia coli como principal agente difiere de otros reportes, pudiendo asociarse al contexto socioeconómico y cultural de nuestra población. Las resistencias reportadas y antibióticos de primera elección en asociación a su evolución posterior son variables a las que podría darse seguimiento teniendo este estudio como parte aguas para observar el comportamiento y patrón de los microorganismos detectados, siendo posible implementar conforme sea necesario mejoras en el manejo de los episodios de FN. INTRODUCTION. Febrile neutropenia is the most frequent and transcendent complication. Institutional reports on infections epidemiology must be consider to choose initial treatment, having in mind variability in bacteria isolated through time and the appearance of multiresistant microrganisms. OBJECTIVE. The objective of the present study is identifying type and frequency of isolated microorganisms, its sensibility, and type of culture and diagnosis with positive cultures of pediatric oncology patients admitted. METHODOLOGY. All cases of febrile neutropenia admitted from 2016 to 2022 were included. Data were analyzed through descriptive statistics using SPSS v24 software and inferential analysis. RESULTS. 580 events were registered, with 44.1 % of males and 55.9 %. ALL was the most frequent diagnostic, followed by AML 21.5 %. There was a 16 % of positive blood cultures reported which align with results from similar studies. In the results compared by year there was a predominance of positive results in 2018 and 2019 with 24.7 and 24.2 % and 2021 was the year with the lowest percentage with 8.7 %. Urine culture reported higher positive results 40.9% followed by periphery blood cultures 31.2 %, central blood cultures 16.1% and secretions cultures 8.6 % bronchial secretion cultures 2.2 % and biopsy 1.1 %. Gram negative bacteria were the most frequently reported with 86 % and 13 % of gram positive with the most reported being Pseudomonas aeruginosa with 22.6 %, Escherichia coli 24.7 % and Escherichia coli BLEE + con 17.2 %. CONCLUSIONS. The studied group reflected similar characteristics in age and predominance of diagnosis according to other reports. The 16 % of positive cultures was along the usually reported 15 to 20 %. The majority of gram negative bacteria isolated with E. coli as the most frequent agent differs forms most of the studies reviewed which might be associated to the socioeconomic and cultural context of our population. Resistance reports, antibiotic used and outcomes were not feasible to include which could potentially be variables to consider in future studies having the present information as a breakthrough to follow the behavior and detection of bacteria involved in febrile neutropenia and implement improvements when necessary. Resumen
Introducción: Tanto la preeclampsia como el COVID - 19 son condiciones de salud que afectan a las mujeres embarazadas, lo que las coloca en un grupo de alto riesgo. Por lo que se realizará este estudio para describir el comportamiento de estas dos comorbilidades, estos resultados podrían contribuir al cuerpo de conocimientos existente sobre la relación entre COVID-19 y complicaciones relacionadas con el embarazo.
Objetivo: Describir los resultados perinatales en aquellas pacientes positivas para COVID -19 y que cursaron con preeclampsia
Metodología: se realiza un estudio observacional, descriptivo, transversal, serie de casos, donde se incluyen a las pacientes positivas para COVID - 19 y preeclampsia.
Resultados: se analizaron un total de 39 pacientes, de la cuales 5 ( 12.8%) presentaron enfermedad hipertensiva y solo 2 ( 5.12%) cursaron con preeclampsia.
Conclusiones: los resultados perinatales de las pacientes analizadas en este estudio difieren con el reportado en la literatura.

Introduction: Both preeclampsia and covid - 19 are health conditions that affect pregnant women, which places them in a high-risk group. Therefore, this study will be carried out to describe the behavior of these two comorbidities, these results could contribute to the existing body of knowledge on the relationship between COVID-19 and pregnancy-related complications.
Objective: To describe the perinatal results in those patients positive for covid-19 and who developed preeclampsia
Methodology: an observational, descriptive, cross-sectional case series study is carried out, which includes patients positive for COVID-19 and preeclampsia.
Results: a total of 39 patients were analyzed, of which 5 (12.8%) presented hypertensive disease and only 2 (5.12%) had preeclampsia.
Conclusions: the perinatal results of the patients analyzed in this study differ from those reported in the literature.
